Coconut cream is a richer version of coconut milk. Good quality coconut cream is made from pressed coconut meat and has a thick creamy texture. It is typically available canned. Some brands simply use a thickener such as guar gum to thicken coconut milk.
Don't confuse coconut milk with "cream of coconut" which is a sweetened mixture used in drinks and some desserts like flan.
Coconut cream: 2 tablespoons = 60 calories and 6 grams of fat
Coconut milk: 2 tablespoons = 22.5 calories and 2.25 grams of fat
You can find coconut cream in most Asian markets. Thai Kitchen is probably the most highly distributed brand in the U.S. Chaokoh is a very good Thai brand if you can find it.
